00 Request for Funds • Consuming Fire Ministries

Please fill out all fields, making sure your answers are clear. Requests for funds to be used for ministry must be submitted at least 30 days in advance. Funds will be approved no more than 4 times per calendar year for a particular ministry. It would be helpful to submit a full ministry budget instead of small requests throughout the year.

A request for funds may be denied if the funds are not available or if it is not clear how the funds requested will be used to directly meet the goals and vision of Consuming Fire Ministries. This form is not to be used to request funds for personal use, such as utility payment, for yourself or another person. Assistance for personal needs is available through LIFT Inc.

You must submit receipts for all funds spent. Failure to submit receipts for ALL dollars spent so will impact your ability to receive funding for any reason in the future.
